investigation-biopsy for histopathology and culture,sensitivity for fungus,nasal endoscopy,CT_FACE
treatment-debridement,palatal prosthesis

Possible DD could be
Midline granuloma,post operative defect,syphilitic gumma orpost irradiation necrosis.
Take detailed hystory,diopsy from the edge and serology.

Possible DD could be

Palatal obturator
needs biopsy, fungal staining and imaging. DD includes midline lethal granuloma, mucor, lymphoma, and rarely tuberculous. please let know the final diagnosis.

Hb%,TC,DC,ESR, Nasal Scraping and KOH smear,Nasal wash and Fungal culture , Blood- sugar preferably fasting ( Rule out DM), Blood VDRL,
Nasal Endoscopy and Biopsy (careful on taking biopsy) CT- PNS, CT-Base of Skull, Audiogram.
If it is confirmed proceed with disease clearance ans Antifungal Chemotherapy, Repeat DNE if required CT-Base of Skull. Keep Inform about this case later .
