Possible DD could be
Midline granuloma,post operative defect,syphilitic gumma orpost irradiation necrosis.
Take detailed hystory,diopsy from the edge and serology.

  3. needs biopsy, fungal staining and imaging. DD includes midline lethal granuloma, mucor, lymphoma, and rarely tuberculous. please let know the final diagnosis.

  4. Dr, it looks Fungal sinusitis of Nasopharynx and Para nasal sinuses.
    Please you may investigate him for -
    Hb%,TC,DC,ESR, Nasal Scraping and KOH smear,Nasal wash and Fungal culture , Blood- sugar preferably fasting ( Rule out DM), Blood VDRL,
    Nasal Endoscopy and Biopsy (careful on taking biopsy) CT- PNS, CT-Base of Skull, Audiogram.
    If it is confirmed proceed with disease clearance ans Antifungal Chemotherapy, Repeat DNE if required CT-Base of Skull. Keep Inform about this case later .
