LEARNING ENT
"Being an ENT Person we have to see all type and kind of patients, right from intrauterine and newborn To Oldest and even dead patients, simplest to deadliest situations, like itching in the Ear to Ventilated Moribund patients.....now...What an ENT-Person should Know more than the Core ENT?: Like KNOWLEDGE OF ANAESTHESIA ,PEDIATRICS,MEDICINE,NEUROSURGICAL ISSUES,OPHTHALMOLOGY,PLASTICS,and DENTISTRY ISSUES..so that whenever in Dire need of the SAME,when the CONCERNED PERSON is not available or not competent to Handle the situation,We can have an edge over What we are just an ENT person.....Share Please...." How to Do the New CPR1. Call help or ask someone else to do so. 2. Try to get the person to respond; if he doesn't, roll the person on his or her back. 3. Start chest compressions. Place the heel of your hand on the center of the victim's chest. Put your other hand on top of the first with your fingers interlaced. 4. Press down so you compress the chest at least 2 inches in adults and children and 1.5 inches in infants. ''One hundred times a minute or even a little faster is optimal 5. If you're been trained in CPR, you can now open the airway with a head tilt and chin lift. 6. Pinch closed the nose of the victim. Take a normal breath, cover the victim's mouth with yours to create an airtight seal, and then give two, one-second breaths as you watch for the chest to rise. 7. Continue compressions and breaths -- 30 compressions, two breaths -- until help arrives.

Modified Lothrop Procedure or Draf-lll along with a complete Sphenoethmoidectomy
VIDEO
Modified Lothrop Procedure or Draf-lll along with a complete Sphenoethmoidectomy is the surgery of choice for recurrent allergic polyps & Fungal sinusitis.Widely opening up all the sinuses, not only enables complete removal of the disease process, but also lets the post operative douching reach all the corners, so that the antigenic stimulation is brought down to the maximum thus drastically reducing the recurrence rate. I present to you my first case done 3 weeks back in this video which is on U Tube http://www.youtube.com/watch?v=A039Xc-fL_Y&feature=youtu.be This sure is revolutionising the management of these untackled conditions! Comment of the Day
LEARNING ENT
The best part of being an ENT is that almost every procedure can be learnt by Cadaver Dissection. Cultivate (?bribe) some mortuary fellows, medical officers etc and dissect on fresh cadavers. If possible keep a separate portable suction etc in your vehicle along with the instruments and dissect, dissect, dissect. Then observe as many people as you can (videos, live, mini fellowships, observerships etc.) and again come back and dissect some more (applying what you've seen). And never stop dissecting. Some of the most pioneering approaches in ENT were (and indeed still are) first conceived, honed and practiced on cadavers..... Dr Yogesh Jain

Montelukast in the treatment of allergic rhinitis: an evidence-based review.
Abstract Cysteinyl-leukotrienes (CysLTs) are endogenous mediators of inflammation and play an important role in allergic airway disease by stimulating bronchoconstriction, mucus production, mucosal oedema and inflammation, airway infiltration by eosinophils, and dendritic cell maturation that prepares for future allergic response. Montelukast inhibits these actions by blocking type 1 CysLT receptors found on immunocytes, smooth muscle and endothelium in the respiratory mucosa. Initially developed as a treatment for asthma, montelukast has more recently found use in the treatment of allergic rhinitis (AR). We conducted a systematic review of studies that have evaluated montelukast in the treatment of seasonal AR (SAR) and perennial AR (PAR), with and without concomitant asthma. Primary consideration was given to large, randomised, placebo-controlled, double-blind clinical trials in which AR endpoints were assessed and the use of concurrent treatments for AR was excluded. Eight such studies were found in the literature. The primary endpoint in these was daytime nasal symptom severity represented by a composite score derived from individual self-ratings of nasal congestion, rhinorrhoea, nasal pruritus and sneezing. Secondary endpoints have included these individual nasal symptom scores, additional scores for eye, ear and throat symptoms, the impact of rhinitis on quality of sleep, global evaluations of outcome by patients and physicians, and measures of the severity of concomitant asthma. A general outcome was that patients treated with montelukast had significantly greater improvements in their symptoms of SAR and PAR than did patients who were given a placebo. As monotherapy, montelukast exhibited efficacy similar to that of loratadine, but less than that of the intranasally administered corticosteroid fluticasone propionate. The use of montelukast in combination with antihistamines such as loratadine or cetirizine has generally resulted in greater efficacy than when these agents were used alone, and in some studies has produced results comparable with intranasally applied corticosteroids. In patients with AR comorbid with asthma, montelukast treatment has resulted in significant improvements in both, compared with placebo. Montelukast is well tolerated and has a favourable safety profile; adverse events have occurred at similar frequencies in patients taking either montelukast or placebo. Montelukast provides an effective and well tolerated oral treatment for allergic airway inflammation in patients with SAR or PAR without asthma, and in patients in whom AR is comorbid with asthma.

Julius Lempert ... Father of Modern Otology
The Julius Lempert story occurred in the early part of the 20th century. It would be an impossible accomplishment in the 21st century. The son of Mischa and Anna Lempert, Julius was born in Lublin, Poland, on July 4, 1891. Not much is known about the boy’s early years. Most accounts start at age 13 when Julius joined a student protest march, was arrested by the police, and thrown in prison. READ MORE

Dear Friends,
We have great pleasure in inviting all of you to the mega event of the ENT fraternity of the South India, AOI 8th Southzone and 31 st AOI AP State Conference to be held at “A Convention Centre” Vijayawada from 12 th to 14th October 2012.
When the Southzone was concieved 7 years ago at informal meetings of some of the senior members everyone was wondering whether we can sustain the energy flowing. Successive and Successful Conferences all around the Southern states of Andhra Pradesh,Karnataka,Kerala and Tamil nadu including Pondicherry proved that always good Conferences are well attended. The last Southzone Conference at Kozhikode proved this point with 900 delegate registrations. The Governingbody and the steering Committee headed by Dr.Mohan kameswaran (2006-2009)Dr.Dwarakanatha Reddy(2009 to 2012) had left no stones to streamline the activities of the Southzone ENT fraternity.
The three day extravaganza includes several events Orations by eminent international and national faculty, Panel discussions,Invited talks and Award and Goldmedal paper presentations by the Postgraduates and Residents and Consultants. We have confirmed acceptance by some of the well known faculty and we were also waiting for the some more acceptances. We are also planning to have cultural activities along with the traditional Andhra hospitality.
Please mark the dates 12-14th October 2012 for this mega event. We assure you an academically stimulating and enjoyable Conference at Vijayawada. The weather at Vijayawada is just pleasnt and beautiful. We request you to attend in larger numbers and make the Conference a memorable one.

Workshop on neurotology,13th 14th and 15th of April,Kolkata
BROCHURE
Dear Doctor,
This is to inform you about my next workshop on neurotology and medical audiology specifically for doctors practising in ENT and neurology which I have been conducting every year for the last 12 years. This is the only workshop of its kind in this part of the world and is designed for clinicians who manage patients suffering from balance disorders and hearing disorders (vertigo, deafness and tinnitus). This year the workshop will be held in Kolkata on the 13th 14th and 15th of April. Prof Harman Kingma from the Netherlands and Prof Gorgio Guedetti from Italy both of whom are international celebrities in the field of neurotology are the invited foreign faculty. In addition we will be having some Indian doctors -neurologists and ENTs who have excelled in diagnostic and therapeutic neurotology and have special interest in vertigo as faculty members for the three-day rendezvous on contemporary neurotology.
The purpose of the workshop is to update clinicians on the diagnosis and management of vertigo and hearing disorders and evolve an ethical and judicious consensus on management. The management of balance disorders has been the most controversial and screwed up subject in contemporary medicine and an ethical consensus needs to be evolved putting all our heads together as many stalwarts in the subject will be present there. This is one of the objectives of the workshop. There will be enough scope of practical demonstrations of the various high-tech investigations for vertigo and the different liberatory manoeuvres and physical therapy for management of balance disorders. All the latest investigative modalities that are now considered essential for diagnosis of neurotological disorders like VNG, ENG, VEMP, ECochG, VHIT, Subjective Visual Vertical, CHIRP BERA, ABRIS, ASSR etc and the ethical and ideal management of balance disorders will be essence of this workshop.
I will be extremely grateful if you join us for this workshop and also circulate the details of this to your friends and colleagues. A brochure for the workshop is attached along with for your esteemed perusal.
You may get back to me for any further details that you require or ring up my secretary:- phone numbers + 91 9831477573 and +91 9836002580.
