3RD ANNUAL CONFERENCE U.S.A.O.I. (Uttrakhand State Branch of A.O.I.)


3rd Annual Conference of U.S.A.O.I. was held at Haridwar on 5th 76th April 2008, (Dr. R.S.Chauhan- Organizing Chairman , Dr. Jasprit Sing –Organizing Secretary, Dr. S.P.Singh- Treasurer), about 50 E.N.T. Surgeons attended the conference. Guest faculty was- Dr. A.K.Agarwal (Delhi), Dr. Naresh Panda (Chandigarh), Dr. J.C.Passey (Delhi), Dr. Sandeep Kumar (Delhi), Dr. Ombir Singh (Ghaziabad), Dr. Ravi Meher (Delhi), Dr. Saurabh Varshney (Dehradun), Dr. Pratima Gupta (Dehradun), Dr. D.M.Kala. (Dehradun). Scientific session included guest lectures on Facial nerve, anatomy of temporal bone, ossiculoplasty, mastoidectomy , national deafness programme etc, Panel discussions on – Advances in FESS, Pitfalls in mastoid surgery , Head neck oncosurgery. New executive committee was elected after General Body meeting.

C/o Dyspnoea,stridor -- 3days.
No dysphagia,change of voice.
Mild to moderate Stridor.
Mass Rt.side of neck 5yrs. Soft to firm mobile neck mass in the thyroid region.
?Enlarged Lt.lobe/ expanded thyroid cartilage. No nodes.
I/L -- normal mobile cords; hypopharynx. Pinkish mass seen in subglottic area.
Xray Neck – Soft tissue mass subglottis,upper trachea.
Xray Chest – Lt.Pleural effusion.
CECT Neck – Multilobulated mass Lt.lobe of thyroid with extension into subglottic & trachea Lt.side. Enhancing mass Rt.lobe with central hypodensity.
CT Chest --- Loculated Lt.Pleural effusion with collapse of Lt.Lower lobe.
Other Haematological,biochemical tests wnl.
24/5 Pleural Aspiration – Frank pus – Cytology No malignant cells; Culture Sterile
AFB –ve.
FNAB(23/5 -322/08) – Rt.lobe – Colloid; Lt.Lobe – Cellular smear s/o Papillary Ca.(Follicular variant)
TFT(24/5) -- FT3 -- 1.23(1.8 – 4.2)
FT4 – 1.40(0.8 – 1.9)
TSH – 0.33(0.4 – 4)
Thyroglobulin -- 76.1(<55)

Thyroid scan(26/5) – Photopenic area(?Cold nodule) Lt.Lobe; vascular cold nodule Rt.Lobe.
Pleural tap has been done & she is now on ATT.
Total Thyroidectomy is planned.
How to manage her Subglottic part of papillary Ca?
I feel that tracheal resection is not possible as the tumor is extending into submucosa of Cricoid cartilage.
One option is endoscopic shaving of the tumor with a microdebrider.
Should the larynx be stented after that?
Can the procedure(tumor shaving) cause subglottic stenosis?
Intra-tracheal mass is too large to be ablated by Radio-iodine.

60 yr old male, has been having dysphagia and chokes on fluids and solids for about 2 yrs or more. The daughter who happens to be our nurse, says it is a pain to watch him taking food.
His X-ray neck lateral shows big osteophytes between C2-3, C3-4, C4-5, C5-6, and C5-7. The ones C4-5-6 looks like a single unit. They are big and measures almost the antero- posterior diameter of the corresponding vertebral body.
A CT done 2 yr back revealed nothing more than this. We couldn't do a Barium swallow for flimsy reasons, but it could be done about a week back, and since the software to record the same didn't work, we had to be contented by just seeing what was seen during fluoroscopy. The barium went down without any hitch, negotiating the "C" projection of the C4-5-6-7 oteophytes at the root of neck.
We sent him to our Orthopaedician, who asked for an MRI; but it couldn't be done, as he was unable to lie down for the same because of pain (Radiologist told the nurse-daughter) Hence a repeat CT was done (Is the position for CT different from MRI?), which showed no change from the previous CT.

What next? I am posting photograph of the X-ray Neck Lateral and relevant frames from a CT scan.
